Kansas UHV Frequently Asked Questions

Question: When does data need to be entered into DAISEY?

Answer: All client and visit information must be entered into DAISEY within 3 days of a visit.

Q: My program uses IRIS for referrals – how do I document that in DAISEY?

A: If you are entering referral data in IRIS, make sure to enter the Family ID into the Caregiver Profile and indicate that a referral was made in the Kansas UHV Visit form. Learn how to find the Family ID in IRIS.

Q: How do I update information on a referral made in DAISEY?

A: Edit the Kansas UHV Visit form to update the referral outcome data. Referrals should be closed and marked as “Organization could not be reached at follow-up” after 30 days if no outcome information is available. After 30 days and referral information was updated (even if unknown) the home visiting case should be closed.

Q: Do I have to complete required screeners in DAISEY?

A: Yes, basic screener information must be recorded in the Kansas UHV Visit form in DAISEY. You can also optionally complete the full screener for the EPDS, UNCOPE, and Social Determinants of Health, if you would like to have all screener information in one place.
